An elderly woman presented with incapacitating chronic cough. Routine chest films and full-chest tomograms showed no abnormality, but bronchoscopy revealed diffuse inflammation of the trachea and bronchi and her sputum contained Mycobacterium tuberculosis in large numbers. Antituberculosis therapy was followed by resolution of symptoms and bronchoscopic abnormalities.
