A case of multiple myeloma with massive pleural involvement is reported. The diagnosis was made by the finding of high protein content, high specific gravity, and many plasma cells in the pleural fluid. The diagnosis was further confirmed on examination of the pleural biopsy specimen (by Cope needle). Protein electrophoresis of pleural fluid revealed a sharp-spiked peak in gamma globulin.
