A rare case of isolated idiopathic right ventricular endocardial calcification with outflow tract obstruction, organized pulmonary emboli, and subsequent right heart failure is presented. Diagnostic features, possible etiologies, investigation and treatment are outlined. This case is believed to be the first of its kind reported in North America.
